Time doesn't matter. It happens every time I run a dungeon

Steps:
This mostly happens with Enemies and fellow Players. They will vanish off the screen and I cannot target them at all nor see them. This usually results in either the party getting mad at me or awkward explanations. if I open my map, I can see where they are, but in the mini map and on screen, their true location are no reflected correctly. Sometimes an enemy or ally will still be seen but will stop moving all together. Everything else around them moves correctly. As long as their true location is close to the misplace location, I can use skills as normal. In most cases, I cannot do this. Now, after the latest patches, I've been noticing things are getting worse. NPCs aren't appearing as fast and the run of Tam-Tara Deepcroft was a total nightmare. None of the enemies were located correctly. The PCs weren't show on the full map nor did they even appear on the screen. When I use Return, it normally fixes, but this time, I used it several times, died and other tricks that normallly apply a temporary fix did not work. I cannot play if the game is going to be like this. This game requires playing dungeons and with other people. It can only be soloed for so long. I really need this fixed and I will not stop until it is. I really enjoy this game when it is not freaking out on me.
