Steps:
1. Get two pieces of equipment that have the same name, they can have different stats, different soulbinding levels, etc...
2. Register one of those in one of your gear sets of equipment (let's call it set #1)
3. Change equipment set to another gear set that does not use that equipment piece (let's call that set #2)
4. Go into your armory and switch the gear positions so the piece you want equipped is located further down the list than the secondary piece that shares the same name that you do not want equipped.
5. Change to gear set #1. The piece you did not want equipped is now equipped and the piece you registered on your gear set #1 has not been equipped. The game basically looks for the first equipment in the list that has the right name, but does not save the actual equipment piece into the gear set.

Note that step #4 is not required to have that happen, it can happen naturally through equiping other classes gear sets. This bug is especially annoying when you have several pieces with the same name for soulbinding/materia farming purposes, or when you carry several pieces of pink random generated gear that, despite sharing the same name, do not share the same stats.
