Steps:

1. Achieved "To Crush Your Enemies VI" achievement on 08/16/2013
2. Opened up titles list on my character sheet.
3. Scrolled up and down several times looking for awarded "the Exterminator" title, but to no avail.
4. Spoke with NPC Jonathas selecting the option to "share a story" (or something similar...) but said title was still unvailable.
