Skill Bar Re arranged AGAIN!

Once again, i had my skill hot key bar changed by the game, when i went into the fifth dungeon, the Halatali....one I had carefully planned my key bar and practiced with them as they were and when i logged into the dungeon and finished the short intro video, and got ready to lead our party into battle.....

things were ON the bar i did not want, like Auto Fight, and things were MISSING i NEEDED...like Flash!!!!!!!!!!!!!!!!!!!!!

this is the second time this has happened,..and it is a real pain, as it interrupts the flow, costs the team time and make me look really slow or stupid...it happened AGAIN when i was sent back to QuarryMill trying to kill Treants, when i got there once AGAIN my skill bar had been changed so that is three times.

Place: In the Fifth Dungeon Halatali
When: immedately after logging into the dungeon with the Duty Finder, after the vid intro, i found at least seven of the skill buttons had been changed, some between the 1 and 2 bars, and some were gone completely or placed there without my choosing them.l

Repeat: By QuarryMill, same things happened again, only with just a few...mostly the Rage of H was gone.... after being sent to the crystal after the Treants killed me

Bug: Skill Bar changes without cause. this happened over a month ago or more, when i was in the Santosh dungeon, ALL of my skills disappered from the bar that time.

Do you have anything preventing HOTBAR.DAT in "C:\Users\{pc user}\Documents\My Games\FINAL FANTASY XIV - A Realm Reborn\FFXIV_CHR~" from being written?
Like a read only flag or any security application? Or are you on PS3? I would add some info asked in the report template.
