Buff icons not wearing upon depletion

Stoneskin/Regen buffs are not wearing off my targeted allies after they have completed their duration.

Very often. Happens about 2-3 times a dungeon.

Steps:
Not sure exactly what the process it is that I'm doing to have this occur.
Generally seeing it when I buff my tank with Stoneskin before battles. As far as regen not wearing off after its duration is up its a lot more uncommon than the Stoneskin bug.

Example in order showing the tank taking damage while stoneskin buff is still up. Pics were taken only seconds apart. I did not recast stoneskin at any point between photos. When asking the tank he too can see the stoneskin buff on so that kind of ruled out possibly my internet connecting causing this?
