Steps:
For the first instance:
I queued for Titan hard mode with a friend from my Free Company hoping to beat him and progress on our relic quest, we went through two groups before we managed to kill him. After he died and rolled for the staff of crags, we left the instance only to notice that Both of our relic quests did not update. Thinking it was just a simple glitch that happened I reported the bug in game and moved on to attempt it over again. Two days later, I managed to find a group that was able to kill titan, after rolling on the holy key, i quickly checked to see if my quest updated and it did not. So, I quickly turned on x-split broadcaster and recorded proof, as well as taking plenty of screenshots.
