Multiplicative 10% on piercing damage; e.g. all direct damage for DRGs and BRDs. Unconfirmed whether it affects the direct damage of Windbite and/or Venomous Bite depending on how they're coded.

If combo: finish combo.
No combo: if HT buff < 3s, HT
No combo: if PH = no, PH
No combo: if CT < 4.8s, ID
Else: TT

Edit: Should should roughly result in the HP IDC TTT / HP TTT TTT rotation. Exact times may need to be tweaked.

[*]Does using fracture break combos.

Any on-GCD ability will break the combo.

[*]Is fracture DoT affected by disembowel

No DoTs are / are supposed to be affected by Disembowel.

[*]Is disembowel counted as a "buff" when applying DoTs, does it affect DoTs as they tick or at time of initial application

No. Fail position is a fail combo; this applies to HT -> ROT as well.
