Same here, as well as a tank who seemed to have it a lot:

Steps:
1.Fight, blackout
2.Error code 90,000
3.Error code authentication issue, code 5003
4.Had to re-launch the game, but I got straight back in.

I had the same... but on my PS3

Time: somewhen around 3PM and 9PM CET
Frequency: once each
World name: Phoenix
Character name: Asteria Odina
NPC name: None
Monster name: non for 3PM, no clue for the other anymore
Class/Level: Dragoon Lv35/36
Party or solo: 3PM: solo, 9PM: party
In-game time: 3PM-ish, 9PM-ish
Area and coordinates: I don't know anymore
Steps:
1. at 3PM: walking to destination (by foot not on chocobo); 9PM fighting monsters
2. I don't know about error codes anymore but showed the following both times: Authentification error, disconnected from server message, black screen (no loading sign, music, whatsoever)
3. I forced the game with controller's "Quit Game" to quit but instead my PS3 beeped 3 times and restarted itself (usually QG brings you back into PS3 menu)

Same thing happend to me, which froze my ps3 as well.

Since enough people have submitted detailed reports, here is some additional information:
- it seems to happen in a timespan of about 3.5-4hours
- it doesn't matter if you are active or not at that time,
- nor what you are doing
- nor where you are

It's like your session simply expires. Maybe someone forgot to code in a check whether the player is active or not?

5:15 EST, October 16, happened again. Almost the same time it happened yesterday.

Black screen, 9000 error into 5003 error, game shut down on its own.

this really seems to be true I had my last 90k 5003 at 21:07 logged back in at 21:30 and now I had again a 90k 5003 and its 00:30 <_<

today a friend logged in right after me. Later, I got kicked with a 90k, and once I logged back in, the said friend got hit by a 90k as well. Just a few minutes ago, I disconnected. Logged back in and lo and behold, my friend disconnects as well (after finishing crafting the botanist AF for me, luckily). So it definitely seems to be a set span of time.
