Camera/Character is shaking

Steps:
1. Walk up/down an incline or even terrain. Character name, character, camera shakes/vibrates violently. If I stop moving, everything is fine. It only happens when my character is in motion.
2. Ramp leading to entrance/exit of Limsa Lominsa.
3. Extremely Frequent around Ul'dai.
4. I have tried EVERY graphical setting from very low to very high.
5. I have tried EVERY camera option.

i get this almost anywhere its not totaly flat, in first person its the world that jerks, in 3rd its charactor.
and no i wont fill out your irrelavant form.
