Trading bug: Item disappears after completing a trade

This bug happened to me on 2 seperate occasions. I will make a report for each one

Character name: Sho Lhung (receiving item) and Azuma Rosinya
NPC name: N/A
Monster name: N/A
Class/Level: Goldsmith/50
Party or solo: Party, I think it was 2 people
In-game time: 10:53
Area and coordinates: Gridania, in front of the market building
Steps:
1. I traded items to craft HQ Goldmsith Boots to Azuma Rosinya
2. Azuma Rosinya crafted the item
3. Azuma Rosinya traded the item to me
4. We both accept
5. I couldn't find the item anywhere in my inventory, armory, retainer, melding window. I am 100% positive that I did NOT discard the item

Steps:
1. I crafted the HQ Vanya Hat of Casting (my own mats)
2. I traded Tyrus Iceshifter
3. Tyrus Iceshifter puts 6xPotash in his trade window
4. I put the Vanya Hat of Casting in the trade window
5. We complete the trade
6. I received the potash, but Tyrus did NOT receive the Hat. The slot where the hat was is EMPTY
7. After check his inventory, i suggest we trade something to see what happens.
8. Tyrus opens a trade window
9. The item APPEARS in the same slot in MY inventory (I have 6xpotash AND the hat in my inventory)
