Frequency: This is the first time I've noticed this happen; I've done a few more synths since using the same actions (it's my standard start for level 50 & below synths) and haven't seen it recur, though it certainly may have happened before and I didn't notice.

I'm going to attach a screenshot with the log to make it a bit easier to see, but I'll explain the steps after the image:
0. Started synthesis (of Cobalt Cuirass) with the following ability sequence:
1. Inner Quiet
2. Great Strides
3. Steady Hand
4. Innovation
5. Advanced Touch (success)
6. Great Strides
7. Advanced Touch (success, condition was good/red)
8. At this point in the synth, the Innovation effect should have been expended, after being in effect for steps 5-8 (Advanced Touch, Great Strides, Advanced Touch). Additionally, the Steady Hand effect should have one step remaining, after being in effect for steps 4-8 (Innovation, AT, GS, AT). However, as you can see from the screenshot, the Innovation effect still has one step remaining and the Steady Hand effect still has two.
9. I noticed that the quality gain for the first Advanced Touch was lower than I expected, so I was curious whether the Innovation ability had an effect but didn't reduce it's number of steps remaining (or the display of steps remaining was wrong) or whether it had no effect at all. Comparing the value to the second Advanced Touch wasn't useful, because the condition was good for that action so the quality was increased. In order to check, I performed the following sequence (still on the same synth) to clear my current Inner Quiet effect and repeat the initial sequence.

12. Great Strides
13. Steady Hand
14. Innovation
15. Advanced Touch
16. This Advanced Touch, which did appear to proc all abilities correctly, resulted in a quality gain of 653, as compared to a quality gain of 464 for the first Advanced Touch. So it looks like the Innovation (and probably Steady Hand) abilities probably weren't in effect at all for the first Advanced Touch.
In case it happens to be useful, my ARM stats are 316 Craft, 324 Control (332 Control with the Raw Oyster I was using), 334 CP.
