Join the large group of people complaining about lagspikes. There have been numerous thread about it in the technical support section. I'm still having issues with it, to the point that I took an almost week long break from it. Because I couldn't farm tomes or complete some quests that require me to fight something...

You are by far not alone, a LOT of people are suffering from constant latency issues, making the game more or less unplayable, and the problem is at SE's hands, or rather their CA data centers.
This problem has lasted for soon two weeks, and for many its getting worse as time goes by.
At the moment 1.0 servers where a lot more stable which is realy sad considering 2.0 would have new server structure and all that.

Oh no, how horrible. 190 Ping, world is ending!snip
On evening it got worse with 190 average and 6% package lost (at least this evening). I don't really know what to do anymore. SE won't explain what is happening neither. Also I have play for years with PSO2 on JP servers without have any problems. Ok different game but still. Anyway wish to hear some suggestion from some SE ppl here.
Why dont you come on over to Australia, where the best possible ping i can get to an MMO in NA is 250 - 300.
unplayable ...
Ok thank you, now that I know it I have the solution. Come on, if you want flame someone change thread. I posted some test for prove im getting what I have been talking about until now L A G S P I K E. Yes, is unplayable for me. How should I say otherwise, when you try fight something, everything stop and after 3-5 seconds you see either him dead, you dead or everything went fucking itself?
If you call this "playable"...
Sure wish I had the average 190 that the OP has. My average if I'm not using my phones internet from peak internet usage hours on my base is about 675ms+ and my phones wifi tethering I get about 273-300ms. Hasn't stopped me from clearing Titan HM either, well using my phones internet that is.
Even if is good ping, why I still getting spikes? That's the question. Also, doesn't matetr to me if have 100 or 300 average, if game lag spike is unplayable even if you have 10 average! Also I'm not complaining about Titan, since I cleared it, I'm complaining about is impossible play now on evening, and is evening that friends start play.Sure wish I had the average 190 that the OP has. I understand you're not complaining about Titan I'm just saying that calling it unplayable because of certain lag spikes is a bit over the top. I've personally have not noticed lag spikes on my server even when it's supposed to be peak play time/ramping up to be peak play time in the states. I'd maybe check your ethernet ports, maybe the cables are going bad that you have. Could even be something with your ISP if it's peak playing time in game it wouldn't be too far fetched that the ISP for your area is getting hit heavily too for internet usage in general. What type of internet do you use is another question. DSL your wiring might be going bad in your walls which can lead to having packet loss.Even if is good ping, why I still getting spikes? The problem you need to understand is this...(I'm bolding this not because I'm angry but to put attention to this part)
Everything worked fine for everyone from FFXIV 1.0 till FFXIV Beta->Early launch. The lagspikes suddenly appeared for tons of people out of nowhere and is only affecting FFXIV:ARR. Which again worked perfectly until a certain date.
I for one don't understand how it suddenly became an ISP issue when my and others people connection worked flawlessly. And I also don't understand why S-E isn't adressing the elephant in the room. It would be nice if they clearly stated what we should be doing to fix this issue. In my case, it even seems to be getting worse over time. The last few days, I had R0(lagspikes) that kept leading into d/c, rinse and repeat.

Then everyone would have broken ISP? Come on, I said I can play fine any other MORPG like PSO2 in JAPAN without a single lag, and could play before as well. Sicne you never noticed lag spikes, you don't know how is unplayable the game. I hope you never get any of it, since getting it every damn evening when ppl trying log in (if you dc or reboot game you will notice queue for wait to log) is absurd. I don't know, ppl want always protect SE, it's always fault of players to me for certain ppl, but what if is SE PROBLEM? Also, I can't afford to change ISP, and I don't see why I should since until weeks ago was working fine. As someone pointed, I seen the thread with lot of ppl having same lag spike problems. So, is me or is SE? My friend changed to fiber and still getting spike lags that he had to use Pingzapper... also why community rep can't give any info about it? I know you reading Camate or whoever else.
