    If the form has no entry fields just put the Server, Date, Time, X/Y map location coordinates, Name of char[s] reported and whatever the form asks of you. Type /echo <pos> and it will show you where you are or look at your map and it will show where you point your cursor on the map.

    Also on the support desk tab in game is a Q& A that allows you to pull up a query form for your question and some sample questions are there. You might be able to ask your question there too.
