This could easily be fixed by having a dropdown where we select the monitor ID for the game to be on when we select borderless window mode, or have an offset X/Y for us to manually put in where the game should render ([-1920, -0] for instance would place the game back on the monitor I need it to be on)

Issue 2: No resolution options while in borderless window mode
When I use borderless windowed mode I cannot use any resolution aside from the resolution of my desktop. This causes and issue when I have my desktop set to 5760 x 1080 which defeats the purpose of being in a windowed mode for me. I would like to be able to be in borderless windowed mode, yet set the resolution to 1920 x 1080
This isn't exactly what's going on. The "monitor" selected is actually the GPU to use. If you have two un-even GPU's (eg Radeon HD 7770 and Intel 4600), the rendering takes place on the device selected, even if it's displayed on the other device. If both "monitors" are the same device the game needs to 'know' the logical resolution to display at. "Borderless windowed" mode is to full-screen one monitor at whatever the fullscreen resolution is set to.

C:\Users\YOURUSERNAME\Documents\My Games\FINAL FANTASY XIV - A Realm Reborn\ffxiv.cfg has some additional things you can adjust. It's not gauranteed to fix your problem, but there's indeed separate windowed and fullscreen resolutions.
