To those that don't know, you have to complete the Living on a Prayer quest. This gives you an achievement (under quests, inbetween the Job and Hamlet ones). Once you have the achievement, go to Derwin out at the Coffin and Coffer, NW of Camp Horizon to claim the item.
For later summoning perhaps?
