As the title states, I would love the option for our character to become either a vampire or a werewolf. It would need to be something that isn't forced upon the player but I think it'd be cool if there happened to be a werewolf and vampire NPC who would offer to empower our character as part of intricate side quests.
One of the playable races in WoW is essentially a werewolf and you can become either a werewolf or a vampire in ESO so there's definitely a precedent for such things in the MMO genre. Then, of course, there's a bunch of single player games that allow for it as well.

On a more semi-serious note, werewolves and vampires are not races; lycanthropy and vampirism are typically known as sicknesses wherein the original host or 'true/head werewolf or vampire is shrouded in mystery. I bring this up because SE is a bit known for digging a little deep into the origins of many concepts they use in their games, and if either were introduced into the game, I am guessing it would be in some form of illness that an NPC is suffering from, or instead of a new race, they could possibly be new jobs. The latter of which I would gravitate to like a moth to the flame. -

ESO has it yeah, but it works in a game like ESO where you are meant to be switching skillsets constantly, on top of having a limited amount to use at a time to begin with. In FFXIV, the werewolf form would probably just end up being "cute", and the vampire form, I don't know what it would do really. Anyway, I already got my werewolf costume from like 2 years ago I think. I can look like a half furry anytime I want!

Final Fantasy Tactics S vampire was a job class in it. Final Fantasy V I think had town of werewolf's in it. So could see them sometime adding them as a new beast tribe maybe.

I think the best way to implement this would just be to make werewolf and vampire beast tribes that offer new cosmetics that would let players RP them more effectively.
To add actual mechanics that are only available to players who choose to pursue this "optional" content by turning their WoL into something else just sounds like bad game design. -

Wasn't the npc that you started the dark knight quest from close to or was a vampire. I'm pretty sure a lupin is a werewolf that's already in Storm Blood.

I mean, we could roll them into the class that people have been asking for, for a while now, a shape-shifting one based on Vincent. I mean, Chaos in dirge of cerberus looks kinda vampirey, and his other form, galian looks pretty wolfish.
As for how it fits in the lore? Easy. We already had vampire-like stuff in the RDM quest, chalk to up to the aether stealing voidsent, and we already have fates involving a dude that turned into a wolf-thing on the first.
Not a huge stretch by any means. -

Vampires die in sunlight and werewolves go berserk and are overcome with blood lust during a full moon. Neither of those are traits that belong on a player character.
Unless you want to spend all day hiding indoors or sleeping in coffins as a vampire only able to play at night, no.
Unless you want to lose control of your character for 3 nights out of every "month" of in-game time as a werewolf, no.
Those traits only belong on NPCs.
