Assuming the child spirit concept farther then it may go, then they could all just be hard-er to tell the difference like young children. Or they're magical and they don't have a real specific gender. Certainly some of the VA is done by females though, with no attempt to sound like a young boy lol.

I wondered as well but then I just thought maybe they didn’t have genders and was going by king because they liked how it sounded. *shrugs*

Given how Faefolk are born in FF14 lore, it seems unlikely that they really have a perceived gender. The only human norm they keep depends on which type they are -- Pixies just want to have fun, Fuah are envious of the things they find interesting, the Nu Mou just want to be of use to people. The Amaro are only 'Faefolk' due to the fact that they reside in Ill Mehg and have gained some mutations, though their love seems to be their strong desire/emotion.
High King Margo can out-fab all of them.
Pretty sure it says in one of uriangers books (I think when you enchant/animate them?), that the pixies adopted that name from the king of the Kingdom of Voeburt, and used it for the ruler of all fae folk. So they simply don't know that king is the "male version" of a ruler, and just see it as the title for a ruler.
The expansion dropped and people are still asking weird questions.
They more than likely don't really have that concept of male/female titles. They just take King as top dog and that's it, to them they are likely thinking why care?

King is a title, usually used for male, but sometimes it can be used for female. There's been female kings in history. Just treat it like a title.
Pixies are androgynous or genderless in my opinion, I wish it was clearly stated if they are males of females because I just get confused by them, even their voices take abrupt changes which only makes it harder to know what they are.
Some player was asking last night if Titania is a trap (transvestite) because is not easy to tell if he/she is male or female.
King is just a title for those trickster fairies!...
Pixies refer to each other using "singular they" pronouns, so it seems they are genderless.
The term 'king' used is explained in the side quests of the zone. Basically it is in reverence to a past ruler.
They refer to their ruler with NB pronouns
I think you've said all there really is to say on the matter. The pixies don't think about gender and are androgynous. I think they look more feminine, but there's nothing that really confirms it. In the end, the fae seem to have chosen king to have been the title of the ruler, regardless of gender.
I play in JP audio and they all sound pretty female to me.
But I don't think it matters in the end. A title is a title, just like a name is but a name.
I think the pixies look like tiny Omega-Ms, actually...

I believe both the name of Titania and the title of King are both in and of themselves used in a way to cancel each other out, gender-wise.
Titania appeared in Shakespeare's play, "A Midsummer Night's Dream", wherein "Titania" is "Queen" of the Faeries. So in essence, the "Leader" of the Fae has no specified gender, and merely appears as a genderless child.
Also if people start calling Titania a "Trap", its gross and disgusting.
