Can you explain what you mean by "My graphics appear to be stuck on high quality?" Do you mean that when you change the graphics settings they do not change in real time?

I don't have any screenshots of the previous graphics to compare with, but things seem to be more detailed than it should be on lowest graphics e.g. stars in the night sky, extra particles on items such as Ixion's horn that do not appear on the lowest quality. When changing graphics to the highest possible setting, these features remain the same. However when manually switching to the lowest possible settings in game, the some of the settings do not appear to change from the high quality settings.

When exactly did this start happening? Did anything change/update on your PC that coincides with this?
You can try to reinstall/update your graphics card drivers. If this does nothing, you can try a reinstallation of the game.

I have already reinstalled the game and updated all graphics drivers with no change in the problem. This seems to have started when my borderless windowed mode changed from 1600x900 to 1920x1080 on both DX11 and DX9.0c with no other changes/updates to my PC.

Nope, FFXIV.cfg has Read-Only unchecked and has the same problem with Read-Only checked as well.

The problem began three days ago when pressing Windows + Down minimized the game with no other changes or updates to the PC. Before the problem, I was able to change settings and I have tried deleting the cfg file and reinstalling with no success.

From here, I would suggest doing a clean boot, simply open the task manager, click the 'Start Up' tab and disable everything, then restart the PC.
Make sure not to run any other programs or overlays before starting the game. See if you are able to change the graphics settings.

Morning, I would also like to report that my graphic settings have also been reset to high performance. Don't know why, as I have literally just rolled out of bed, turned my pc on and the game had all my graphics on high.
I sat and fixed everything, logged out and logged back in and thankfully everything has stayed the same. Apart from the loud noises of the fans from my pc, but I'll get someone to look at that later today.
So in short - also reporting that I had the same issue. Hope you get yourself sorted Cool75739 -
