I'm pretty sure that was released a while ago, the one most people are after, is a table that says which items turn into which materia, not a table of materia by slot.
"Below is a list of the materia that can be created from each source item type and slot."

Actually he's right, this chart is old and has been posted several times before, though it was never officially announced. Also, it does only show you what TYPE of item turns into different varieties of materia (i.e. Body Piece can become any type of Bloodthirst), rather than showing which specific item becomes which particular materia (i.e. Toadskin Harness can become Bloodthirst II +x amount of HP). The former has its uses, but the latter is really what the community was hoping for, although expecting that level of information to be given away by a company as secretive as SE was probably overly optimistic...

I'm willing to bet that chart shows the literal information. What I mean is, /any/ type of materia can come from /any/ specific kind of gear in that slot represented. For example, it doesn't matter if that belt is for DoW, DoM, DoH, or DoL, it will still produce, at random, any of the materia specified on that chart.

Yes, but players have noticed that certain gear tends to produce certain specific materia, while other gear in the same slot will tend to produce a different materia entirely. There are definitely some trends as to what item makes what, and knowing what those trends are would help people make more informed decisions about whether they want to turn their 100k gear into materia or just sell it, for example. However, it's understandable that SE doesn't want to just hand us this information, but when a materia chart was first announced there were hopes that this is what it would be.
