October was the last time I played on my PC. I bought two CE at launch.. To my dismay the game didn't run very well on my PC, it ran acceptable but I decided to wait for the PlayStation 3 launch.
Anyway, what the biggest changes since then? There were a lot of things wrong with the game back then..

Depends on your definition of "wrong" rather than buggy. The game's UI while still serverside is 100x more smoother, it's quicker to level with SP changes (it's consistent like Exp gains) and so on, basically foundational fixes.
They're in the process of changing the foundation down to even classes, so still a lot of big changes happening every 2 weeks. -

The biggest change since oct is they shut the servers down the other day so you cant play now like you could back in oct.

The two biggest changes for me are the new targeting system, and the ability to repeat synths on local levequests. You can now choose specifically what you want to target; party, enemies, etc. And there's a hell of a lot less button mashing when crafting now.
They also added "mini quests" recently, and although lackluster, they're still new and offer something else to do.
I guess the biggest change of all would be the among the devs. I'm not sure if the OP knows or not, but almost all the top dogs on the production staff were fired/quit in December. The new team is MUCH more sociable and the director, "Yoshi P" as he is known, has been very forthright with information about upcoming patches and whatnot.
Oh and yes, the fatigue system still exists. :( -

Mostly just very small tweaks so far. They added quest but they're like leves without sp from killing the mobs.
The major changes haven't happened yet nor do we have any dates for when to wait for them or know what they are. -

I'm surprised no one mentioned the increased size of monsters. That was a major change for me.
Alongside the repeat synthesis, inventory sort and aggro icon above monsters. ;P -

Nice community we are building guys..............
they fixed some animations to speed up gathering and such.
they fixed the sp so leveling is a tad easier both in parties and solo
sp is no longer by hits.
They increased the size of some mobs to be ff esk
they added and changed a market ward item search, and lets you mark the retain to find them faster
they added side quests for hint lore purposes. Its not anything direct.
They added an Aggro Icon to agressive mobs, but you still need to learn how to avoud them
they changed passive/active combat mode switching and added a light visual on the mini map.
Npc have a tasteful ff esk ! so they are easy to spot.
levequest are now easier to get a quest from based on rank.
they changed the crafting window to be a bit more seemless.
Thats all I recall from reading link and watching youtube vids as I'm waiting for ps3.

they have made quite a few changes and imo some were good and some were bad, but i think your question is the biggest thing they have fixed. to me the biggest fix they have made since october was you no longer get the "cannot find directx device" error every 5 minutes.

There have been a lot of little changes here n there. You could probably say things like: Monsters are double in size, SP is static is maybe biggest but they all tweaks/small adjustments when it comes to game scale.

Glad to see people coming back to see what's going on. Kilta covered most of the basics so far but the reduced lag in the menus is the best to me. I use a mouse/keyboard combo and the lag in the menus made me not want to play before. That is pretty much gone for me now.

Biggest changes since October, eh?
I'd go with the new producer / director Naoki "Yoshi-P" Yoshida as the biggest.
But in the game itself, probably a vast reduction in lag would be the first thing you notice.
There have been a lot of subtle (but mostly good) streamlines in the UI. Things involving repairs are a good example (totally changed for the better). Still room for improvement, but getting there.
The way you gain SP in combat has changed (works like XP now).
I'd say those were the biggest things. -

I took the OP's question to be more like asking which changes had happened that were the biggest and most important seeming to us (since there have been soooo many!), so here are things that come to my mind . . .
- Crafting: more streamlined process. Way fewer clicks!
- Repairs: repair icon added, makes getting repairs so much easier. Also durability has been increased a ton for items that are the correct rank/class for you. Overall repairs are not a big pain like they used to be.
- Markets: you can search the wards now, plus you get two free retainers instead of one
- SP gains: they have tweaked it more than once, but it seems to be in a good place now IMHO. Parties are definitely favorable.
- Notorious monsters: a few have been added. A fun thing for those at/near the cap to to do in groups.
- Population: sadly it is quite small compared to how it was in October. And if you're not in Thanalan good luck finding people to play with! ;_;
