Since we're in 3.5 right now I think they will come avaible atleast at or after 4.0

4.1 - 4.5, you mean. The freedom of Doma and Ala Mhigo will not come instantly, anymore than it was to end the Dragonsong War for Heavensward (patch 3.3).

Actually being able to traverse to them immediately? This will probably occur within the release 4.0 story, before patches.
I doubt they would keep a region advertised in the trailer locked on launch. The story will probably take us to both places and continue in Doma during future patches.
My best guess is one or both of those towns will be end game towns that you will end up rebuilding..

The answer is.... Yes. That is what the entire expansion is predicated around. The liberation of Ala Mhigo and Doma. It would be very difficult to liberate them without going to them. The story to do so will begin in 4.0 and carry us through the patches. As to the exact timing of getting to them, the answer is "We don't know yet."
I would assume it would be the same way Ishgard was in HW.
I think they'll be available, but I'm not sure in what form since Kugane was added. They do have an increased budget so 3 cities isn't completely unreasonable. My money would be on Doma, having been razed and all, will be a part of the Yanxia zone as ruins a la The Dravian Hinterlands and the Sharlayan colony. That being said, I hope all 3 places stay relevant in some way during the 4.0 life cycle.
Sadly, as with HW and Ishgard, there likely will be the "one town" where everyone will eventually congregate. This will most likely be whatever location the Rowena shops for tomestone turn-ins will be. They attempted to keep Ishgard relevant by having the hunt mark billss and Temple leves only available there, but after a certain point there's no reason to do those so Ishgard became a ghost town. I expect this cycle will sadly continue indefinitely through all the expansions of the game.
Hubs are normal, yes. There's no problem with hubs, in all reality, but that doesn't mean there has to be one hub, is all. I actually think that having the 2 cities is a great opportunity for SE to make either hub equally as attractive for players to go to so it's not just, "This is the last place I got to and it's the only one with the end game content and vendors so that's where I'll always end up going." At the very least doing that could help alleviate the congestion and ease the problems people have with loading and frame rates that they currently have with Idyl.
I'll be curious to see how the patches progress. I imagine 4.0 will end with both regions being, 'Technically,' liberated. Imperial leaders slain, imperial forces in disarray. The two patches after probably would involve the installment of new leaders and governments, and bringing said governments into the Eorzean Alliance. Probably have to deal with a counterstrike by the Empire in the 4th patch, and then the 5th patch has us seeing the Garleans set their eyes on a more valuable prize, a nation with the knowledge and resources that would allow Garlemald to easily conquer the planet when combined with their Magitech; Sharlayan.
I suspect both will be in 4.0. However both will also, at least to start with, be occupied so wether we can actually use them as cities is yet to be seen.
I think our war with Garlemald proper will be over by the end of 4.0 and Garlemald will have its own issues with an impending war of succession.
Just basing this off the launch of Hw but I would imagine that through the starting msq's we'll get access to Ala mhigo, as for Doma being added in getting access to that may not happen right away just depends on how the msq's flow
If you mean being able to go to them, then yes, you will be able to go to them in 4.0. You will not need to wait for a patch.
But the story regarding them won't be finished in 4.0.
I wouldn't count the Empire out as of the end of 4.x...SE needs to keep them relevant as an enemy for 5.0 and perhaps beyond.
Who said they will be the enemy in 5.0? Its not like we are going to completely destroy the empire. If we did it would turn Ilsabard into a chaotic, war torn nightmare. Most of it has been Garlean for nearly 50 years. I think its more likely we will try and keep Garlemald from completely collapsing and try to put a more moderate leadership in charge.

What I meant to say was by the end of the initial content we could make it to the capital, still likely inside a resistance hq through the msq's.
Now if they go back to the format of ARR we could go to all areas and explore them. If they keep the format from hw where you have to progress through the msq's to unlock each new area map then hopefully through the msq's we'll be able to get each new area unlocked in the initial content like they did for hw.
I believe all of the zones they have previewed will be accessible in the 4.0 patch. Notable locations we have been shown include Rhalger’s Reach, Kugane, and Yanxia (where current Doma resides). During the latest Fan Festival it was suggested that they would be doing something unique for the liberation of Ala Mhigo. For that reason I am not entirely confident that the physical locations of Ala Mhigo will be accessible in the 4.0 patch.
Worst case scenario:
Doma = FFXV's Tenebrae (staring from the outskirts)
Ala Mhigo = FFXV's Insomnia (basically a dungeon or raid)
Best case scenario:
Doma/Ala Mhigo = Ishgard
