I play on the PS4 & I've been having an issue where I will LB in a dungeon without meaning to. I do not have LB on my HotBar currently (This issue is happening on my low-level healer) so I'm really confused. Looked at macros/etc and could not find any preset LB setting I need to change. Google didn't yield any results either. I feel like I'm going crazy :) anyone have this issue too? :(

Do you have Extended Crossbars enabled? If so, you might be jumping to whichever crossbar is extended for a split second as you jump between the left and right triggers - if LB is on that bar, accidental use can happen. :p
Either way I'd go through -all- eight of your crossbars and make sure LB isn't on any of them. -

You do know LB is party wide? So when anyone in the party uses it, the gauge empties on everyone. Sorry if this was obvious but sometimes the out of the box answers are overlooked.

I'd check your keybindings too in case you've somehow accidentally bound LB (or a macro you don't normally use with LB in it) to a key combination. It's difficult to imagine how that can happen by accident, but you never know...

What Sida says; When someone in your party uses Limit Break, you'll be tied to the animation of cheer (you can still move and do what ever you're doing when it happens) and you'll hear the sound that it is currently going on. If you're not sure who's using it the moment it happens, look at your party list and it's the slower casting action with a special name. Braver, Bladedance, Big Shot, Desperado, Skyshard and Starstorm being the most common as they're Limit 1 and 2 from dps.

Also, I briefly mentioned this, but this is mainly happening in my low level dungeons as a 30-ish healer. Both WHM and AST. I only have BLM and BRD in the 50s, and neither of them have had this happen.

That's interesting. One time-consuming thing you might want to try is to back up your character settings and then reset them to the defaults to see whether the issue still exists. First go to Settings> Application Data> FFXIV then back up every file in there somewhere safe, like a USB drive or PS+. Then, delete the file with the same name as your main character from the PS4's drive. Please be very careful to make sure you backed up successfully before doing this or it will break my heart ;_;
When you log in again all of your filters, HUD adjustments, macros, hotbars and gearsets will be gone and you can quickly set up a new WHM/AST configuration and head into a dungeon to see whether the problem is fixed. If so, that definitely indicates that the problem is somewhere in your character settings/hotbars/macros rather than a weird glitch.
After the experiment you can restore your original settings from the backup file. Or if you haven't made too many customisations and the wipe fixed the problem, you could just keep using the new save file and gradually redo all of your macros. It took me about two hours to remake mine after my data was corrupted at the weekend but if the LB issue is causing frustration it might be worth it.
