I thought this profile background image was for getting all six achievements in Foundation Day, but I've done that and I seem to still be missing access to it. Was it form something else?

Hmm.... I managed to get the three-flag background, and I'm pretty sure I only got three Foundation Day achievements done (I got the bonus seals from all three grand companies). Maybe the site just hasn't updated yet?

Have you completely exited the game since you got the achievements? It isn't something that happens instantly. Completely exiting seems to help speed up the process.

How exactly do you pick up the bonus seals? I got 1000 from the Immortal Flames seal vendor but do not know why or how.

The single flag is becoming an exclusive member for a GC.
The three-flags I have no idea. I'm having the same problem here. I rather soon believe it has nothing to do with getting all the seals, or shooting off all the fireworks, rather than blame it on a system bug.
