But... Cash shop... Ugh.
I agree. We could spend all day in the sun and get a tan in real life, so this kind of option should be available in the Aesthetician. (never heard of tanning beds?)

I'll give to that point, but considering the vast majority of the Eorzean populous don't have access to Magitek Technology, this is pretty much a null-point.
Eorzea is a continent seperate from Garlea. The Garleans don't like the backward, simplistic nature of most Eorzeans and see us as savages basicly.
So, it's pretty transparent walking through Ul'dah, Gridania or Limsa that Magitek Technology is pretty much non-existant.

TL;DR Main Reason - They won't add it, they're making mad-dosh off of Fantasias.
Please point out where magic was available in the pre-renaissance era, please point out where death wasn't final, where people could be hit by a plethora of arrows without bleeding... Please point out where potions that changed your race and gender were available and so on...
The real reason is what Shioban pointed as "main reason", they are profiting from that.
Feel free to blame cash shop supporters.
Fantasias being a premium service predates the current cash shop. They've been a premium service for much of the game's current lifespan.
And it makes sense. You get two of the freaking things for free already, if you want more you gotta pony up the dough. It's the least intrusive usage of premium services the game currently offers.
Would this not be possible for Cid to get done? He has the knowledge to build from Magitek armor, and we know he can also make adjustments to it to account for supplies he can't get (the automaton core in the Magitek armor you get for example.)
It's not like you haven't ever had to gather items from the Garleans as well.
Random fun fact about me: I wasted my first fantasia because I misread the text for the OK button, but I had hit cancel. What was my original intention for using it anyways? I had no idea why I had a tattoo on my character's back and none of my friends knew either (downside of being on a non-legacy server I suppose :D) and I wanted to get rid of it.
Fantasia as a veteran reward has been available for a while, but fantasia as an item for sale has not been up for "much of the game's current lifespan", it was added in 2.28 due to player feedback, they had to use the veteran reward system to distribute them because the cash shop was not ready yet, they always said the bundles we had before were temporal until they had another system to sell them separately, they were always meant to be a cash shop item implemented early due to feedback.
All I'm saying is, people shouldn't expect any new feature addition to the aesthetician as long as fantasias are being sold.
(also we only get one for free)
It is unfortunate, but like already been stated, they won't probably because they advertise using fantasia as making 'minor changes to your character's appearance' in addition to making huge ones.
I would have loved for them to have been able to allow eye-color changes (contact lens, eye-drops, what have you), but I ended up fantasiaing to go play Hyur for a bit. And changed it back on my to Lala. It's a pity since it's close to $10 a pop, but people are willing to pay for it.
I can see it now... that weird Elezen skins you alive and stitch a fresh skin on you after he remove the fresh skin off a Ul'dah refugee...

I dunno... lore wise make-up and hairstyles are easy to explain away. Changing your entire skin and eye colour overnight makes zero sense. He's an aesthetician, not a sorcerer!
^and replacing tattoos on our face so easily makes sense lore-wise too. So does equipping on a tattoo from armor.
If you wanna go there, lore-wise, we could have fruits (it is a FANTASY game) that alter the pigmentation of our skin.
They could implement it to where we lock our character in the Inn Room for 6 hours, feeding him/her 40+ fruits, and praying to the RNG Gods we get the Skin Color we want :)

Anyway, the major point being that not everything has to make perfect sense lore-wise (at least, not something superfluous as skin-color change)...
And I said "so easily" meaning 'with ease', alluding to how it's said & done in a flash.
Explain how the process takes place instantaneously if it's supposed to make sense.

Yeah, if he can remove our scars and give us new ones I don't see why we can't have fake tan and contact lenses.

I think it would encourage people to pick some of the more unique skin tones that are rarely used. (dark red-tan, gray, blue etc)

Please point out where your average Eorzean citizen has access to this technology.
They just don't. Eorzeans are simple people.
Ignoring all that, from a development point of view, the only thing they're changing is possibly adding in eye-colour adjustments, when skin was last mentioned they declined as it simply wouldn't be feasable, especially with purchasable Fantasias on the horizon.
I'm not saying it's a bad or good idea, I'm saying from a lore and business stand-point they will never add this.

Every MMO offers this service, it's a basic practice.
Every 1.0 character have a free redesign available at launch without Fantasia.
first free Fantasia came from the veteran bonus
second free fantasia got also registered to every 1.0 account as a bonus item because there were some problems with the character redesign at launch.
But it would be cool if we can dye our skin. I would use metalic gold on my character ^^
I think the skin color should remain in the realm of Phials.
...Now on the other hand, I DO think that Miqo'te tail "styles" should be an aesthetician option. Not the length, just the style. It just another hair option where a species has visible hair on more than just the top of their heads.
Stop using lore to justify this, you're looking silly, you very well know lore isn't the reason why none of this will be implemented, it's the good money they are getting from phial of fantasia.
You can't paint crevices into the skin. Unless he's using his scissors in some way...Oh! Then if they're just illusions, let's just use glamour prisms to change the color of our eyes! Because magic.
Look, the point is you're looking for lore reasons to deny it, and it's silly in this fantasy game. If they're gonna deny it because they want money from fantasia sales, sure. But let's not pretend it's for some semblance of lore, shall we?

Oddly enough, Yoshida said a while back "Contact Lenses" would be a feasible excuse (During a Live Letter) to add it to the Aesthetician, whether or not they actually add it, is a completely different story.
But disagreed to changing skin-tone or face-shape.
I'd honestly like to see SE meet us in the middle on this one.
We should be able to change hair color, style, eye color, skin tone in game at the esthetician. Miqotes should get the option of tail changes. Maybe make the eye color, skin, and tail changes more expensive then other procedures so that it is a gil sink.
However, if we want to change race, gender--then make that cost a Fantasia from the cash shop. I don't mind paying for those major changes, but its things like paying a cash shop to change something minor like eye and skin color that make me start to feel like I'm being price gouged.
