Seeing people who you just unintentionally carried through something think they're really good and try to farm said content they got carried through...
People who geared up solely through Hunts that think they are the best in the game...but can't keep up with a raider with "worse" gear who knows how to play their job well...
Tanks in CT who think they should MT, despite having a lot less gear and dying as OT in the previous fight...and the aggro wars that happen there :(
That one enemy that decides it wants to aggro you once you interact with something. (Syphs in the Ramuh story, looking at you. >_>) I bet they're really annoying during those beast tribe quests.

Can't really think of anything game mechanic-wise that bothers me other than Heavy from behind having a 20s duration.
But what is really ticking me off lately is some of the tanks not hold threat off a single Cure (not II), or Physick. I've had two noticeable instances of this lately. Earlier this week I had a tank in crafted gear, no belt, no jewelry tanking in Stone Vigil that just could not hold threat off my faerie. Those mobs hit terribly hard and once the faerie goes down, so does alot of the healing.. and of course I'm the next target and get two shotted. Then just as of 10 minutes ago.. I was queuing as my WHM and did a Cutter's Cry and the tank in full Myth gear and their Zenith weapon couldn't hold from my Cure spells. Not talking about residual regens from previous fights (made sure not to do that).
1: I don't think anything in this game bothers me the most then the horrible decision to have every attack capable of inflicting heavy while mounted, it's bad enough mounts don't even move that fast and then a heavy status you can do NOTHING about, with a long duration AND it can be applied, resetting the timer.
2: Trying to get old content done as a DPS, the long....queues..just..ugh. I grew up as a Healer, so this is just unacceptable!

The treants drop the frame rate when you have them in your screen and the maze like structures is just woe is me.

Players bolting into a dungeon as soon as duty commences. As a healer, i don't like having to stoneskin/protect everyone every time I enter an instance, but I do it because it's part of my job. If you run off and start attacking stuff, I'm fitt'n to let you die
Somehow ending up with an nq item after 98% quality plus failing a t4 materia meld with a 90% success rate.
RNGues is perfect. Nothing can compete against the satisfaction he provides, so great is SE at RNGeus that players make up wacky myths like looking towards the seventh star at dawn while in a house with five candles praying thrice before botching the first two crafts to HQ the third.
Sarcasm aside, I really wish SE would make the non-assured numbers friendlier over time per failure. Purpose is not to make candy pinatas, rather it ensure that each player experiences a vaguely similar work schedule. Could remove RNG entirely of course and use tokens, but sometimes "hope" is more fun than knowing exactly 51 more runs are in your future.
Instances starting while you are aggroed from several mobs and can't change to the job you registered for. After 1 year, still SE? Not possible to auto join a dungeon no matter to what job we have changed?
Having to fight 3 mobs for a beast tribe quest but having to get rid of twice as much on your way there in order to be left alone from the random aggroing mobs that can't really hurt you but annoy you without end.
The beastmen daily where you need to carry items through aggroing areas.... Hate those missions!

Having queued for duty for quite a while then calling my chocobo and getting kicked out of queue. Why can't the game let me dismiss the bird before I join the instance instead?
People showing up in my expert roulette wearing High Allegan gear that don't know how to play their job.
People demanding I heal them in the field or in a fate because they bit off more than they can chew while they are not partied with me or even know me. FYI I just watch them die then walk off. Of course this makes them rage in shout.
People lying in party finder about clearing a raid or trial to join a farming run hoping for a clear carry and loot while being entirely clueless about the mechanics.
People that try to force their opinions on game content on others as if they were facts.

Questing during the low levels then having a fate drop right on top of you then all the mobs swarm you at once because you are the only PC in the area.
Questing during the low levels when suddenly a hunting mark is spotted in the area then having a giant mob run in and start yelling at you because you are not in their LS even though you had no intention of participating and are to low level anyway. (This happened once. It was crazy)

Being queued in duty finder for hours, leave the computer for 1 min to take a piss, come back to see the last 3 seconds disappear on a queue. Set back to the end of the line...
or
being in a queue but doing other missions, queue pops in the middle of a cutscene, you try to skip it only to have the timer run out as you wait for your game to load.
That reminds me of another one.
In Duty Finder Being honest that you havent done "X" before and asking for tips then having people rage on you and disband the party. 3 days after that content was released....
Not being able to do the content cause you have no experience but not able to get experience cause no one will stay in the party once they realize you are learning.
Seriously though, i'm not an idiot. When I say I havent done "X" content it means just that. I know how to play the game and rather than learn on the fly i'm askin for a little leg up like, "this guy casts doom when he roars, when you have doom, stand on the glowing plate to get rid of it." That's all I really need. Too many people overreact and behave like I just bought this character and this is my first MMO and first time playing anything in this game.
The gravity effect of mobs who aggro... they are not dangerous in any way... why they want waste my time? ><

That godsdamned titanic sawfish, the non-existent drop rate for atmas, basically anything that requires dumb luck to complete in a reasonable timeframe. (when your luck is as bad as mine, these things are your anti-existance.)
Not exactly "little" given the cost, but melding and desynth send me into a fit of rage. **** you RNG with your 8 failures at 75% success chance in a row before you finally let at least a few successes through before you fail one more time. **** you RNG with your making me fail a 45% success meld 18 times in a row. But thank you RNG for always being a little nice to me with less spammable events (like Atma and gear).
Oh and being hit with Heavy from a lv40 mob... that's always pleasant.

Other than that nothing really comes to mind that hasn't been said. What can I say, heavy's prettay annoying and to be fair I don't see why it's a thing on your mount. Indeed, your mount exists to get from A to B faster, so why in Bahamut's name do we get slowed by nearly every attack that hits us while mounted?!
