I think that using cooldowns for MB is important. As a healer i try to use adloquium, but sometimes it is not enough, and virus/eye4eye cannot be used for every of them.

That looks more like a 3-stack+ MB; I normally swap at 2-stacks (or when stacks wear off on the other tank) with my OT, but we're never in danger (running 8k~8.4k EHP each) outside of 3 stacks with no CDs up. I'm not saying we don't use CDs or that they're not necessary, but shit happens and Titan's rotations are annoying as hell to track if phases last too long.
From my experience this happens a lot during the 2nd MB after heart (1st bomb pattern spawn -> landslide -> MB), when tank should have 1 stack.
I have few theories, one is about boss damage going up with echo (raw damage was 12589 on sc), other one is about boss skills being changed in 2.2
I will keep an eye if MT ever stands with 3 stacks past heart, because this damage is too high to take unmitigated.

More like a 2 stack with gaolers alive.
The Gaolers give Titan a determination buff (strength depending on proximity, iirc); I've never quite noticed the level of damage they added to MB due to our phase changes being in-consistent (partial static), but I wouldn't rule it out (hitting 2 stacks and having Gaolers spawn is just bad luck).
I'd ballpark it at a 50% damage increase. To the point that you should treat it like it's +2 stacks on yourself.
Or more to the point, that the DPS is bad. If both gaolers aren't dead.
To be specific, you see a yellow tether between Titan and the gaolers as he does Granite Gaol if they are not yet dead; this is your last chance to kill them before buff Buster.

I think you guys are on to something with the gaolers. When I do this fight as WHM, I have to remember to keep the MT topped as I'm dodging landslide/bombs in the final phase because if he isn't topped, there's a good chance he'll die from the MB that occurs at that point. I always just assumed that the reason MB seems to hit so hard at this point is because I am frantically trying to heal everyone else too (I've never looked at the logs), but the gaolers buffing his damage definitely makes sense now that I think about it. It would be a good idea to throw up a virus at that part of his final rotation, to try to counteract the gaoler buff.
I did today solo tank attempt Titan Ex (which failed obviously) but i didnt die even at 5 stacks. Only 6th killed me with MB.
Was it 5 stacks past heart?
if the add alive when he do 2nd MB. (the add will have line link to the boss) depend on your gear it will hit between 10k+ (with no CD use). saw that a few time while i tank him. just pop Holmgang or HG if that happen.
i solo downed titan ex with 10 stacks, but i suppose that is due to how awesome the healers were. Also if dps is good there really is no reason to tank swap since phases can be skipped, i can get away with 1 stack at phase 1 and 3 stacks at phase 2, then 4-6 stacks post heart and 5-8 stacks past superbombs with no tank swaps.
Your stacks should fall off after superbombs before the one he does after gaolers, so you must be very talented to keep them.
If gaolers are alive for too long they "enrage". You know this is happening because they have a beam connecting themselves to titan, enhancing all his damage, and their own defense.
Rule of thumb: have enough dps to kill them by or before Titan has the time to do 3 mountainbusters. 3 is viable on a well geared tank, 4 + gaoler buff is certainly not.
