Lol that's great. One time in Dzemael Darkhold I had to tank, kite, DPS...and HEAL myself. Only two DPS since tank Healer died. Boss at 1/5 HP. I tanked a little, kited a little, DPS a little, then lb. I had to use my second wind, bloodbath (saved my life), and life surge. Barely made it. The other DPS was bard, it was pretty funny though as it seemed so helpless.
Had to clear HM with One Tank, One DPS since the healer DC'd/quit right at the start and the other DD ragequit.
Got a replacement DD and managed to clear to the last boss.. and thankfully a healer joined us in progress at the last 5 minutes.
Is a classic, but...
WP, our tank examines everyone (not great average party ilvl), he ignored us and started to run claiming all mobs without giving time to our whm to cast protect, stoneskin, etc.

2 days ago i joined Brayflox HM as tank, our dps were both melee (ok, no "big aoes", so we'll go cautious). All went fine until after the first boss. At that point i though "mmm, the healer is first timer, so if i take all the goblins, both melees won't have enough tp to kill every mob and probably we will be killed. I'll take 2 groups only". So i did... and the Monk decided to pull all the mobs. They did aoes for a few seconds, then they ran out tp, the healer couldn't handle it and we died. A few seconds later the monk said "sorry, i don't have enough tp to do aoes"

Tank left on our cutters cry before everyone could finish watching the CS. Tanked it from start to finish on my MNK..... New tank finally joined while last boss was at 50% hp. He got credit for the kill haha...
I was the one who made a wtf moment. I joined a duty as a tank and tabbed out for about a minute to check out what's new on the crunchyroll and when i tabbed back, both DD left apparantly because they don't want to wait .... their DD queue must be ultra fast.
Derp moment on my part. Final boss of Cutter's Cry. I ran towards the boss when I should have ran away. Thing is: I knew I should have been running away and I was yelling at myself for doing it. After downing the boss I was like "That was my bad. I don't know what the hell I was thinking. I knew I was supposed to run away, instead I had a blonde moment and ran towards it & made the healer do unnecessary healing. fml" At least the group was understanding (and probably had a good laugh). The healer responded with "Well, at least nobody died from it, so it's all good."
Aside from the derpage on my part, the dungeon went very smoothly.
Back when I was leveling my White Mage to 50, I ran Aurum Vale. A Tank decided to pull nearly everything in the first room, and I couldn't keep up due to having MP issues and having to deal with my own aggro because he couldn't handle them all. (And I even used Shroud of Saints) He tells me to use Aetherflow to regain MP faster than Shroud.

actually monks aoe is not that bad, if you build your GL3 without Arm of the Destroyer (using just snap punch, twin snakes to keep that buff and rockbreaker) cuz its just waste of TP, you can then follow with blood for blood, internal release, perfect balance and rockbreaker spam, those goblins should die before you run out of TP, dont forget invigorate. Tank should be able to survive it rotating his cooldowns you can even help him with mantra.
Queued for expert roulette and got into an ongoing brayflox hm immediately after start queue, they were all at the last boss locking the fight out for me, halfway on the way to the boss, the leftover couerl that they didn't kill saw me and I was on this hit and run one-on-one battle with it as a bard, the three members cheered me on while they're at the bomb phase. I kept running around in and out the narrow path to keep a distant from the couerl because of its aoe paralysis and then managed to take it down without dying, when I get back to the boss right after the BLM dps managed to take down the bomb with LB and then they defeated the boss without me, I got 45 soldiery just for killing a feline.
Believe me or not. This happened to me a few weeks ago in Lich. A friend of my FC went to Aurum Vale, she had to do it for unblocking Amdapor Keep and for the grand company, If I remember right. While I did some roulettes... After two roulettes, she told me that she was yet in the dungeon but that the tank was kicked... I shocked and offered her my help...
When I entered the dungeon, they were in the final boss, with 5:50 min remaining for kill it...Anyway, we tried hard, everything was going well, but the time was against us.... In the middle of the boss I changed to "DPS mode" putting defiance off.
....
...
.....
Only one small little bar remaining in the boss and the time finished. Duty failed... Had to form a new dungeon starting from zero with her for having success in 30 min, like it should be (?).
And I'm only a noob, she too, maybe the tank they had was just trolling them.
Once tanked a Stone Vigil where one of the DPS was a level 42 THM. I think it was an accident; I asked him before we started how come he wasn't a BLM and he replied with "dammit sorry" and then didn't speak another word the whole run. Poor guy.
Also queued in progress into Tam-Tara where the rest of the party was already on the final boss. I watched them win from the entrance. I was basically only there so they could fill and use LB. :(
i have a tendency to pull stuff for the hell of it, also i like bringing the easily avoidable Coeurl on turn 1 into the caduceus battle to see how long i can make it last.
i have a macro that says: "brb, being a drg. <se.1>" for when i am about to do something incredibly stupid now.
I had a major facepalm lolwut moment the other day...
Doing duty low level roulette and titan story mode came up.
When I got in all of them were new to you so I went all out explaining the tactics to them and helped them to memorise what happens during the fight
Things were going perfectly smoothly then he was due to leap ready for heart phase and I went to type in chat (which I looked down to type) heart phase incoming be ready to switch to it.
Looked up to see landslide on its last split second cast on me, needless to say I took a trip through the air into the pit of death.
They beat heart phase just due to one DPS down, and done the fight flawlessly , felt so proud of them since it was their first time but also felt such an idiot for forgetting one vital basic part of the fight lol
SCH in CT stood still for 80% of each boss fight spamming succor till he ran out of mana. He asked the bard for mana song each fight but the bard ignored him (and rightfully so). On the final boss the SCH called the bard a baddie for not playing mana song for him. lolwut
Fresh today: Copperbell Mines (low), slime room. A lancer were killing the spawning caps, even we telling him not to do. The tank were running around all the room as if the slimes were killing machines. Me (healer) and arch go sit in a corner waiting how much time needs before they will understand that hitting the slime boss is pointless, and that does not hit so hard to make tank running as an headless chicken.
For me my lolwut moment is something rather silly. I was doing WP runs and after we cleared the first boss our tank ran into the wall after moving passed the pugs.. I know it's silly, but they weren't on auto run. I have never laughed so hard while playing this game until that moment.. Thinking about it now makes me smirk.
I recently did Halitali (HM) on PLD with a very impatient and annoying BLM that would sprint off and pull the next set of monsters before killing the last set. At one point dying before he could make it back in range of the healer! x)
So when we got to the section with the three Bears that use a fan directional AOE; I pulled all three and started running over to him using their AOE markers to force him to move and interrupt his casting.
He got frustrated.

While doing WP myth runs I encountered someone who used the 'insult' "I'd be mad too if my name was comb." No one in the run had anything close to comb in their name, and the randomness of it made me crack up. My girlfriend and I still use it as an inside joke many months later.
Wow that's kinda scary now that I think about it. Had a SCH doing something similar in a roulette AK run 2-3 weeks back, only he was spamming Adloquium on the tank until he ran out of MP. We didn't have a bard (I was on MNK and other DPS was BLM) so we ended up limping through the last boss. After we won I told him that the galvanize effect doesn't stack and only gets replaced if the heal has a higher potency than the last one. I asked him why he wasn't utilizing Physick and Embrace to conserve MP and he said Physick was weak and that Adloquium replaced it. He then proceeded to tell me to learn to play, insulted the tank, and left. We were all just dumbstruck by the whole thing.
Joined Party Finder advertised as Ifrit EX. The party leader qued for Titan EX and I guess expected a carry from people that already beat it. Ummmm no.
I had 2 moments. One was when I hit 30 acn, I unlocked both smn and sch. So I figured I'd test be a healer. Did a run with my fc on tama and things went well, so I ran by myself. This time I got 2 nice does and a tank who thought it was an amazing idea to pull the first 3 areas of mobs. I managed to heal through it, with him telli me "I should be having fun".
I tanked a Brayflox once with a lvl 33 pugilist wearing a mage robe and 4 pieces of crafter (as in for a crafter, not crafted) gear.
This is nothing that happened inside dungeons but it came as a surprise to me so I hope it fits in here
Two times in one day I managed to queue up for wrong dungeons.
I was gonna do brayflox hm and accidentally queued up for brayflox story mode, and I didn't realize it until we got to the first boss.
Later the same day I figured that my fresh level 50 summoner should try out Ifrit HM, so I queue up for it and find a group pretty quickly
I had accidentally searched for Titan HM though which I had never done before even on my main. Took me some time to realize it too "Hmm isn't there supposed to be like a firewall around this place and OMG THIS ISN'T IFRIT"
So they all just started the fight and I had no idea what was gonna happen.
After a couple of wipes we did it though :D
One time I healed a Toto-Rak run where the Marauder tank was wearing the starter axe, FFXII chest, pants, and legs, caster accessories, and crafting gloves and head. He never said a single word the entire run, kept getting lag spikes, and only ever auto-attacked.
Fortunately, a Dragoon in the party wound up doing the tanking and we cleared the dungeon fairly easily despite basically doing it a man short.
Another lolwut moment, again as a healer:
Queued up for low-level roulette, got Titan Story Mode. Tank apparently went AFK during the opening cutscene, so the first 10 or so minutes of the duty was wasted waiting for the tank's CS to automatically progress. When it finally ended, we waited another 5 minutes before finally kicking the tank, and then attempted three-manning Titan Story Mode.
It went surprisingly well, actually. We got past Heart phase, and if I hadn't gotten hit with a Landslide, we would've won. After wiping due to no heals, another tank finally came in, this time I managed to avoid Landslide, and we won easily.
So yeah, those are the lolwut moments I've experienced so far.
My last oddball moment was in a DF run for Braflox HM. The party was composed of a PLD, BLM (me), WHM, and a BRD. As we were doing our run through I kept pulling threat off the tank (don't really see this too often anymore but it happens), so no problem I thought as I'll just slow down my DPS to help alleviate the problem. Still I would end up ripping hate off the tank even after slowing dps way down and with quelling strikes up. This really started making me scratch my head, so I started paying more attention to what the PLD was doing. Turns out he was rarely flashing, and kept using Rage of Halone outside of combo (Fast Blade > Savage Blade > Rage of Halone) so he wasn't getting the full enmity benefits of the attack. I mentioned to him that he really needed to begin using the combo as he was only getting a fraction of the hate that he could be getting. His response to me was, "I don't need to I'm in Shield Oath." Uhhh okay.
I guess he thought, for whatever reason, that Shield Oath somehow allowed him to get the full hate potential of Halone without the need of the full combo. Wow lol.
my 2nd time was haukke manor tank said it was his first time, so we said we'd help him as we went, since haukke isn't hard. After the first set of mobs, we gently reminded him to use shield throw flash and hate Combos to which he replied "well no shit". I said it would help with him not let mobs get away. He said "well dps is all over so..." we told him he should start marking targets since it will help him focus the group. He got snappy, saying no, there's no time and he's having to much fun. So we told him "Well, it will help you to tank better and get better for the future and Good to learn for your job.
And he told me (being healer) "how about *you* mark them? All your doing is healing." (lolwut?) at which our dps who mentioned he Mained tank propely explained to him that it's his job as a tank to properly mark enemy's. Anyways, tank got mad, left the group, and me and the 2 dps ran haukke ourselves and cleared it with no tank, or even any deaths.
Needed Stone Vigil for my first book for my zodiac weapon. Queued up (on my SMN) and got an instant in-progress pop. I accepted, entered the instance and about three seconds after I loaded in the last boss died and I got credit. I cleared Stone Vigil in three seconds! xD
I was doing low level dungeons for my conj levels, and this tank REFUSED to flash. I'd pull mobs with one cure.
Finally I kindly let him know. "Hey. Flashing helps?"
You know what he tells me?
"I can't flash or they get resisted from it"

I joined in progress for hakkuke hm and saw tht there were only 2 dps in the party left so i was like cool np we'll wait for a tank ( @ last boss)
We still had like 40 mins so i was like u want to try? So we proceeded to fight her but then i noticed 1 of the dps was outside and i was like Wtf!? There was a monk in there fighting alongside our bard .
I was like o.o 3 dps?
After we wiped it was confirmed that he wasn't in our party and was also confused y he got in.
But the lol wut moment was when you still fail the last boss with 3 ilvl90s dps. I was tanking as a 5.5k hp whm.
Not exactly while on duty lolwut but happened shortly afterwards!
I was farming atma in North Shroud while queuing for df and switched out of my zenith weapon when I entered the dungeon. After I was done, I forgot to switch back to zenith and I continued farming for a while. I was soloing one of the easy boss fates when I suddenly realized I wasn't using my zenith weapon. With the boss almost dead at ~1-2%, I quickly swapped in my zenith, and lo and behold! Atma of the Archer dropped! I LOLWUT so hard at that point. I had been farming in this area for 6 days!
Went into a Darkhold dungeon run on my drg while still leveling. The tank takes off before the whm had a chance to cast any spell. He runs through and grabs every single mob and takes them to the room before the eye boss to stand in the blue crystal light. We all fought valiantly but of course with that many adds (other dd was a mnk) we ended up being killed. He gets up and says "come on now which one of you wasn't in the light? were invincible from all damage in that blue light" then takes off again. This time the 3 of us just /sit and waited for him to die again and he immediately left. Fortunately another tank showed up and we breezed through the entire dungeon with no deaths and fairly quickly too :3
Once hopped into a High level roulette pre 2.2 on my alt character (BLM) and got Wanderer's Palace. In the party was a DRG that was wearing some odd gear. During the first pull up to the 6 bugs, somebody messes up somewhere and we wipe, we try to find out what happened exactly, the DRG simply says "Just play"
We get past the first pull and during the second pull (much smaller this time) and stuff is dying incredibly slow, and that's when I noticed the DRG doing an incredibly odd combination of abilities, including Chaos Thrust spam outside combos. I asked them "Why are you spamming CT outside combo?" they simply reply "Just let me play! God!." My reply: "Well by that, I don't have to play with you." Then I ate a 30 minute cooldown. Had a good laugh

I was doing some T4 and learned that my loved SMN is actually a tank and not dps.
We had a wipe and random party member asks me if I have Flash.

Had a tank the other day(new character according to the lodestone, assuming the guy didn't roll a new one just to mess with people) that wouldn't stop running in circles around the one mob he was hitting, thinking it would let him dodge auto-attacks and targeted abilities. Couldn't hold hate on anything, even the one mob he was actually paying attention to.
I enter The Stone Vigil as healer and got paired with a poorly geared tank so was having trouble keeping her alive, and I ask her:
"Can you please use your buffs on big pulls, I'm having trouble keeping you healed" to which she replied: "I like to keep my buffs out of CD so I can use them when I need them"
I was like... "Umm... you need them now !!" >.<;;
Yeah, she had a tendency to die at Dragon Aevis and would only use her buffs at boss fights... and I bet she left with the impression that I'm a bad healer =.='
