I ended up using this. It gets the job done and the latter half of 2nd hotbar [food, alchemist button, blist button etc] could definitely be replaced by whatever. I found it pretty easy to access the first half of my 2nd hotbar using Ctrl+# of whatever I wanted to cast, so I put the not-too common skills there that weren't uncommon enough to put in the later half of the hotbars.
But if I had to give another SMN some advice I'd definitely tell you to make a form of those hotbars I have on the left. They're easy to click when I need to swiftcast raise/summon and it gives me a nice spot to put unique skills like Virus/Eye for an Eye/etc.
Honestly the most important thing in making a good hotbar for a SMN is making sure your DoT spells are in a comfortable and memorable position for you [not necessarily next to each other, whatever floats your boat]. Just mess around with the HUD and remember you can press Ctrl+Home to resize your selected hotbar.
The clicking is strong in here.
Just key-bind things to keys that are most comfortable to you. Personally I had my movement keys set to Q(strafe left), W(forward), and E(strafe right) and do the rest with my mouse. So bind keys around that for instance 1-5 (perhaps 6 depends on your keyboard and reach, I would never bind past 6 though), R,T,A,S,D,F,G,Z,X,C,V, ect. Then you also have modifiers. I'd recommend Shift and Control giving you another 20+ keybinds, some people use Alt but I avoid it. You can also use F1-F4 for some not as urgent or out of combat skills like buffs, raise, food and what not. Also if you have the proper mouse I prefer to bind urgent skills on side buttons like silence.
For the UI set up, just try to keep it neat and near the bottom so it's out of the way. Once you've played a class for a few days and got the rotation down you shouldn't really be putting too much focus on your skill bar, only quick glances here and there to watch for cool downs or procs.

Are all bound to my mouse.
I click on my pots on the bar on the right, but that's the only clicking, and it may very well change when PvP comes out. Swiftcast is in a few macros, I have it up there to see the CD. Same for Eye for an Eye(it's my Alt R).
I just use the vanilla UI. I haven't had a reason to change it.
My only question is, how do I make the pet bar disappear?
Thats my main gripe. I have it off to a corner somewhere and as small as possible.
You can't as far as I know.
I just made mine small and..That's about it.

Hey, @Kevee, you're using Vanya's set? Can you share your thoughts about it? I'm interested in craft this one instead of finish my Darklight :)
Any thing for Pad users? just cant play the game with a KB :/
I use a Controller+keyboard hybrid, The majority of the actions the SMN uses themselves are on the pad crossbar that span 2 sets then I have pet commands on a hidden hotbar then keybound to the numpad like so for easy avatar controls

I then keybound pet buffs like sustain/rouse/spur and also Away on the 1-9 line on the KB, The hotbar above the chatlog has Steady/Guard there just so I can visibly see the stance my avatar is in. Crossbar is set to not appear unless i'm holding LT/RT.

In the Character Configuration window, then in the Hotbar Settings button you can tick an option called "Hide unassigned slots".
You don't have to untick it whenever you want to add a new skill/item to your hotbar, as dragging anything with your mouse temporarily shows the empty hotbar slots.
