Companion dead but cannot dismiss/resummon

Steps:

1: Die whilst fighting Boar Poacher. Companion summoned at time of death.
2: Return to Home Point, and Teleport back to Hawthorne Hut zone.
3: Companion is shown as 0HP (i.e Dead) but does not auto dismiss.
4: Click "Withdraw" returns error "Action cannot be used when Companion is not summoned"
5: Click Ghysal Greens reduces count by 1, and casting bar shows. But Companion does not appear. Withdraw still does not work after using Greens.

EDIT:

Additional information.
6: Able to Disband Party through Social Lists tab. Companion is removed.
7: Click Ghysal Greens again reduces amount by 1 and casting bar shows, but Companion still does not appear.
8: Zoning out/in does not fix the issue.
9: Completely logging out of the game and then back in DOES fix the issue.
