Unable to complete the quest "in a titan spot"

Frequency: um every time..
World Name: Hyperion
Character Name: Briggitte Bellatrix

When i reach the final step of the quest "In a titan spot" which is to attune to the aetheryte crystal to finish the quest and add titan hard to duty finder is when the problem kicks in.

I click on the atheryte and it says atuning, then the on screen message pops up saying "quest complete" "the navel(hard) added to duty finder" but the quest stays in log and titan is not added to duty finder.

It does not give me the option to abandon the quest and no matter how many times i log out/zone/change job etc etc blah blah nothing does the trick.

same for garuda quest exactly the same stuff
