Missing Story Quest

I had the quest to complete the bowl of embers on hard mode. I started the quest and killed ifrit. After the instance i no longer had the quest, nor did i have any record of its completion or even able to get it from the quest giver again. So basically i am stuck story line wise while this quest is missing. I sent a game master bug report to final fantasy's site and they directed me here.
