Will this item be available by other means? or will it be a one-time thing only for veteran 30 day reward?

I think it would be nice to sell it via square-enix store or something.

I was thinking the same.

I've buggered mine up twice now.

Square Enix could fund a division off me alone, just to get my character fixed.

Edit: Yes, your 30 Day Reward is your only opportunity to get one currently.

There was also one given to 1.0 players

I have a question about this item, can you rename your character after redoing it ? it seems silly to be able to change sex and still have the same name

Yes. Check the 'Mog Station' it details the veteran rewards and how they are obtained.
