Did not receive minion for completing Grand Company Hunting Log

I just completed my Order of the Twin Adder hunting log and didn't receive my Mammet #003G minion item. There was no indication that I was supposed to go somewhere or talk to anyone to pick up this item, and I made sure to have open spaces in my bags.

Is this a known issue, or did I just get the short end of the stick?
