Need a fix for autorun on gamepad

I'm having an issue with autorun on gamepad. Pressing the LB button while running will only initiate autorun about 70% of the time. Pressing LB will only end autorun about 90% of the time.
The former is an annoyance, the latter has caused multiple deaths due to running off of cliffs, and Leeroying into boss rooms in dungeons.

I have tried 2 controllers on PC, and 1 on PS3. I have the same issue with all 3. I need either a way to fix this, or a way to map the keyboard/mouse autorun function to my gamepad. Oddly the autorun initiated by the keyboard/mouse works flawlessly.
