Note:
It is possible to craft 50** with only single Off Hand forbidden, just need to meld [Craft 1] on all the pieces that has it last. You can start off this way and add extra Materias as you go. This is what I did but it does not give best value since you will be blowing up rank 3s with 19% success rate rather than rank 1.
Alternatively, you can gain 3 more Control by using [Control 4] on Wrist and both Rings, I didn't think the 3 extra point worth the investment of rank 4 Materias but it might help push a couple more HQ out.

Is HQ Militia a GC item? Haven't heard of this item but I am a newb.
