I would start with Yoshi. It was his project, it is his responsibility. There aren't many businesses that would overlook all of his mistakes and say "bah-no problem - so you pissed off the majority of your fan base because you didn't have the vision or expertise to understand what you were hired to understand". Yeah, I would start at the top on this one.
And what's odd, is that a simple queue system would have spared so much frustration by so many people. If he didn't know how to code it, he could have asked the people over at Blizzard or any other MMO producer. But to release like this, with no idea, no clue, no plan is inexcusable.
