NA - PS3 - CE Registration Code Invalid

I am NA player with a physical PS3 CE box. When I redeem my 12-digit code through the PSN it says my code is invalid. I have tried replacing the 8s and Bs and Os and 0s and etc. -- I'm not dumb. No combination works.

Because I can not register my code, I can not play the game. It won't let me subscribe because I apparently don't have an active account. Now that the EA time has expired, I'm locked out of the servers.

Has anyone else experienced these same issues with the PS3 CE Registration Code?
Is there a fix for it?
Is there a phone number I can contact to resolve it?
Would I need to go through Square Enix or Sony?
