Physical CE Error

I pre-ordered the PS3 Collector's Edition of FFXIV:ARR from Amazon.com and received an early access code and the pre-order bonuses when I registered the game.

Now I find I am unable to receive the 4 additional bonuses in game that come from having bought a Collector's Edition.

Upon checking the Mog Station website, I find that my account type is listed as: Legacy (Win), and it has an option for upgrading to the digital CE for $19.99...

I tried entering the bonus codes included in the PS3 box but the website has 5 list boxes and my codes (there are 2) only have 4 lines.

The codes are in the xxxx-xxxx-xxxx and 6Letter-3Number-6Letter-3Number formats, none of which I have found I can use. =(

It's possible my PSN account is not liked to my SE account... but I can find no information on how to do so, either in the box or through a simple google search.
