To everyone who did not receive a code or got incorrectly registered as PS3.
It isn't an "official" fix, but I canceled my order through SE and then re-ordered it through Amazon. They immediately sent me e-mail confirmation and minutes later sent a working promotional code.
Obviously since even after cancelling the order it will remain a "pending transaction" for a couple days before being debited back to a bank account, this will only work if you have the money to buy the game again- but it worked for me.
