When closed out of the game, hit CTRL+ALT+Delete and go to your Task Manager. Click the tab for "Processes" and make sure you don't have another process for FFXIV running.
A few FC mates of mine were having log-in issues and found they were running two processes of XIV. Ending the process fixed the problem.
