Legacy Player Names and ARR Credits

Hi Reps,

I'm sure you guys probably have some info regarding this already, but I'm wondering whether or not there's a place to request our names to be listed in the ARR credits yet? Will we be sent an email when the request form is available? Or is it something we need to contact SE support about on our own?
