Audio and visual effects lag issue, PC (Windows)

My game audio stops completely and makes my visual effects such as character attacks, character attck animations, enemy attack animations, and abilty visual effects not appear at all for a few seconds, this issue happens both with or without controller, both in and outside instances, both the overworld and any city.

The only hardware that I have connected is my keyboard, mouse, headphones, and a drawing tablent, as well as the aformentioned controller (I repeat, this issue persists wether the controller is connected or not). As for software that is active at the same time is just discord, OperaGX (Browser) and spotify, I have tried playing with all this apps closed, the issue persists as well.

this issue wasn't present before 2 things.
first: me getting a brand new, straight out of the box Dualsense 4, Playstation 4 controller and connecting it straight to the game.
and second:patch 6.45 being implemented.

I don't Know what is causing this issue, but I have tried the workaround that SE and the FFXIV team posted a few months ago of disabling the speakers on the controller, I tried solutions from Reddit posts, from Youtube videos, and nothing seems to solve the issue.
I've even tried uninstalling and installing back the game, in both my HDD and my SSD, this doesn't solve the issue either.

Any error message(s) received?
Are you using a bluetooth dongle via USB 2.0 or 3.0 or on built into the motherboard with a bluetooth card?
If available, have you tried using different controllers?
Do you have any other games or layouts running when playing the game such as Steam, Discord, etc?
Does your connection dip or is the lag related to the connection to the server/internet rather than actual input lag?
