Autoattack intermittently stops working

Steps: (including a workaround)
1. Enter Active mode.
2. Approach monster (camera locked or facing, point blank range, doesn't matter)
3. Wait and see if autoattack works.
4. If autoattack does not occur in 10 seconds:
4a. put weapon away
4b. cancel target
4c. re-target
4d. enter active mode.
4e. wait and see if autoattack works.

This seems to have a chance of happening in any fight. It is especially annoying in primal fights, where target loss occurs in quick succession, causing the issue to potentially happen again.
