Born Again Anima cannot be completed

When turning in Born Again Anima, after selecting the weapon and the newborn soulstone and going through the cutscenes, it says the quest is completed and the weapon has been placed in your inventory but after the screen comes back from the fade to black the quest is incomplete and the weapon is not added to your inventory.
