Stuttering issues

Up until a couple patches ago I don't think I'd experienced anything like this, but recently I've had some weird stuttering going on during instanced areas, normal gameplay runs smoothly for the most part. No factual frame drops or anything of the sort.

I'm currently playing at 1080p on a 144hz monitor with G-Sync and NVIDIA Vsync enabled, exclusive fullscreen, framerate limited to 120 FPS.

I have tried enabling and disabling Fullscreen Optimizations, turning Windows Game Mode on and off and have disabled Windows Update.

Moreover, when I limit my framerate to 60 fps the whole game stutters terribly. Plus, when I tab out everything else running gets a little laggy/stuttery (Browser, File Explorer, etc). It's only with FFXIV that this ever happens. It's really weird.

Again, up until some patches ago I don't recall the game ever acting like this.

I'd appreciate any help. Many thanks in advance.

Tuning in for this, I experience the same with the same setup except being on 1440p/144hz with a 2070 super.

Yes, only FFXIV. I've tried other games and they run normally.

BetterJoy for Pro Controller support and RTSS to cap framerate, as well as Discord, though I usually close it while I play.

Bumping as I've tried the following:

-Turning the Vsync/FPS lock off in-game and turned Vsync on in NVIDIA Control as well as the integrated Frame cap (Alternatively, I tried RTSS too)
-Updating Windows and my graphics driver to the latest versions to absolutely no luck whatsoever, so I've rolled back.
-Played in every screen mode. Windowed and Borderless stutter the most, Fullscreen's a little better with it (The game used to run in Windowed and Fullscreen perfectly before)
-Turning Fullscreen Optimizations off
-Applied lowest Graphic settings
-Tried DX9
-Reinstalled the game (It's currently installed on my SSD)
-Played with nothing running in the background
-Turning Limit framerate when inactive off

Playing at 1080p on a 60Hz Monitor

Have you tried with Icue turned off?

I did yeah. I tried running the game in Windows clean boot mode to verify if it was a third party program causing the problem and still no luck.

I was advised to set power to high performance in both the Nvidia Control Panel and the Windows power profile and that managed to reduce some of the stuttering. Though it comes at the cost of higher temperatures.

Based on the information provided, the issue may be stemming from one of the background programs conflicting the game resulting in the stutter or a particular setting. As a clean boot may temporarily disable the program from running the background, this may still cause the issue. Due to this, we would you like to try testing further by uninstalling any potential problematic programs from your machine entirely then test the game after each instance. As this will be a process of elimination, it may take some time to narrow down what the problem is.

Additionally, you may want to try changing various settings in Windows and nVidia Geforce Experience/Control Panel to see if that has any affect pertaining to graphical enhancements and frame rate limiting.

Lastly, go into the nVidia Geforce Experience software, locate Final Fantasy XIV in the list and select "Details". From there, try changing various settings or use the "Optimize" option to see if this has any effect.

No luck after trying to uninstall my programs.
As I've mentioned before I have tried limiting my framerate both through Nvidia Control Panel and through RTSS, as well as using Nvidia Control Panel's Vsync.
For now, I've found setting my Windows power profile to High Performance/Setting SpeedShift - EPP in Throttlestop to 0 seems to reduce the issue considerably, though I get some occassional stuttering in crowded areas.
