Confess your sins!
Okay, this is a bit tongue in cheek and an attempt to be light hearted look at the various oops we've pulled as Tanks.
My biggest sin is forgetting to check if my stance is on after dropping into sync'd content with my tank...only caused a wipe once...so far...
Other one is a more gunbreaker specific sin, popping our ult on accident(right next to our regen CD on my bar) when I want the regen instead because I'm still finding the button placement I want.

On Rhamu Savage I was single handedly responsible for almost 2 days of wipes.
He's got that move where he dashes across and kills everyone unless the tank intercepts it. I knew to do the mechanic I wasn't able to push buttons. I didn't realize moving counted as a button.
Worst part is... I was Paladin so with Passage of Arms I really should have known better lol.

I used to point leviathan (normal) tank busters at the pug healer if they pissed me off. In fact, I often used such mechanics to murder pugs if they failed to meet my own standards.

One time I was tanking Dun Scaith. On Diabolos I wasn't thinking. When he went to do his tank buster/cleave he was facing the rest of the raid, which was grouped up in the center. Yes, I accidentally killed 23 people in one hit.

I spammed Cure III (+ Medica II running) under divine seal on Mhach during HW once to get aggro for science and make my party and the tanks panicking.
No one even noticed it was on purpose.

Back in HW, when I was still a noob, I once forgot to equip a shield before doing a 8-man fight. Not forget a CD. Forgot the friggin shield, as a PLD.
I don't know how it happend, I just know it happened and I didn't even took notice of it until someone in chat asked "Uh... where is your shield?". I felt so bad, not helped by friend in voice chat laughing at the error, but finally laughed with them. It fortunately was to no consequence as I was OT and it was Gilgamesh, so not a hard fight, but I still look back at this moment as my moment of shame as a Tank. A bit fondly nowadays, I must admit.

If an avoidable aoe does not inflict damage down or some other obnoxious debuff, I will occasionally stand in it and shield through it with The Blackest Night just so I don't have to move.

Pop Bolide on my friend long ago when she just used Bene on me just as I hit it, I got the scolding of my life.
