Steps:
1. A member of the party (NIN) is struck by Barbs of Agony cast by the Shadow Warrior wolf, and is killed as the Umbral Orbs are resolving Shadefire and Flameshadow at approximately 4:00 into the encounter.
2. The Umbral Orbs responsible for casting Flameshadow do not despawn for the rest of the encounter.
3. Further Umbral Orb casts no longer generate orbs that cast Flameshadow on the party, with only orbs that cast Shadefire on the tanks active.

Unsure whether further steps are necessary, as we did not attempt to recreate this bug after seeing it and resetting.
