Instance problem.
Ok, I've had two issues, one was, I died, and friend exited on his own, then the instance didn't end, I stayed in it dead.
The second, a group died, during the blm body fight, one exited, and it would not allow us to return after getting ported back outside. Kept saying "Command cannot be executed right now." each time tried to return

started the fight, few died during, tried to run from instance.
upon exiting the instance, the dead members remained inside the instance, with no one alive that remained inside. this occurred during the holy fight south of the entrance to limsa as well.
even logging out of the game and back in did not boot people out of the instance.

The first one is normal. Everyone has to leave dungeons by themself. A single person leaving won't make everyone leave.
Just use return to get to the area entrance then exit. (this is working as planned based on your description)
I've had that second problem before too though. Seems to fix itself after a few moments but it gets there but it can be annoying.
