Steps:
1. Made weapon hidden
2. Hidden weapon also made hair and ears hidden and left holes
3. Had to log out and log back in to fix the graphical error
4. Changing zones / instances did not help
5. Others could see the effect so it wouldn't isolated to my visuals

1. Recently, changed class from CUL to RDM
2. Instantly lost ears and hair again
3. Entered The Prima Vista Tiring Room and the issue resolved itself
