Windows Cursor appears in game

There is a small rectangle when playing FFXIV, where the FFXIV mouse pointer turns into a windows mouse pointer. Anything that is in that space is not accessible. Has anyone else seen this issue?

How long have you had this issue with issue? Additionally what have you tried to alleviate the problem so far? Does this only happen in Final Fantasy XIV?
